Human Error Greatest Threat to Aviation Safety

AINonline reported this week on an article in The Psychologist by Don Harris. Writer Robert P. Mark notes Mr. Harris believes Dr. Reason’s Swiss Cheese Model is no longer relevant as an accident prevention model because so many of today’s aviation organizations outsource a wide range of functions.
I can’t say I entirely agree with Mr. Mark’s characterization of Mr. Harris’s statements. Nor do I think the Swiss Cheese Model has lost as much importance as Mr. Harris implies. No matter how you interpret either writer’s words, the concept of multi-layered defense against error is sound and we all benefit from it.
